BDS-19232end exit portal kills you
I randomly died when I passed through the end exit portal, the death message said I fell out of the world but all I saw was getting teleported to the overworld finding myself dead. I've searched both the end and the overworld but all my items were gone. not sure why and how it happened.

Please check if this occurs outside of your dedicated server
