MC-6667Blocks are disappearing after being placed in creative mode when using some inventory-management shortcuts
When you have a block on your hotbar in creative mode you open up your inventory and hover over it and press a number(1-9) and the block will move to the assigned spot on the bar. When you now close your inventory, select the spot and place the block it will instantly disappear and also vanish from your inventory however many blocks you had there.

Windows XP SP3, Java: Version 7 Update 9.
