I can confirm that this is an issue in 1.0.4.0.
The 'pack_manifest.json' file has been incorrectly named 'manifest.json'
Upon further inspection I don't think it's a simple as that, as the other RP's use this different format for the manifest.json file as well.
It is a specification that the file name changed.
This issue seems to be merely a mistake of description.
i'm Japanese, so sorry to mistake in English.
Fixed as of 1.0.4.1.
