Crafted pistons. Had space in inventory but piston was not created. Tried it 10 times now. Lost all I tried to craft

Did it also use resources? I've seen something similar to this, but haven't been able to replicate it correctly.
Sometimes when I break down a placed piston block the dropped piston disappears and my inventory count doesn't go up. If I try to place a piston from this stack, the piston will flash for a second, but not actually be placed. If this same stack of "now has added ghost piston" pistons is in my inventory when I try to make more pistons, resources will be randomly used up. I had a full stack of iron and tried to make ONE piston, and it used up the entire stack of iron with no piston going into my inventory. I've found that if I trash that "ghosted" stack of pistons and just start over with "fresh" ones, they make just fine.
In regards to the "ghosted" piston block, I think it may have something to do with breaking down a piston while it is powered. Again, though, I can't figure out the exact replication for this.
