Also confirmed the glitch/bug still exists in 13w04a. Issue occurs once in every ten uses of the command block with /tp command. Stone buttons in the chunk with the "stuck" stone button still operated normally. Crash report located above-
Issue still persists in Snapshot 13w05a. Reproduced the glitch/bug this time by quitting the game while standing next to a normally functioning stone button beneath command block. Then restarted the game and the same stone button was stuck in the depressed position.
Still experiencing the same issue as described on my Feb 1 post using Snapshot 13w09c in single player mode. Experienced two stuck stone buttons in 22 uses.
Experiencing all of the same issues as described above. Running Minecraft (Vanilla) on Mac OSX 10.7.5 Java Version 1.6.0_43
I'm also experiencing this issue but only in the Nether on Creative mode. It also appears Ive got the same message from the DevCon:

I just experienced this very bug(not really a big, imho, but more of a curiosity) several times while using 13w25c. The situation was placing water from a water bucket into flowing water and the rock breaking sound occurs for each water block that has leveled off to the same elevation as the adjacent water blocks.
