Several times I have been unable to respawn when dying in the Nether. I wake up after dying in the Nether (by lava or Ghast fire) in my bed in a Stronghold 1300X and -5300Y away from the initial origin, but I am on fire. I click respawn, but I respawn in the same bed on fire and die immediately (or never wake up
, not sure). To avoid this, I have to quit to the main screen instead of immediately respawning, and I do not respawn on fire and die and arrive safely in the last respawn point. However, if I do not do that, I run into the prior issue. The most annoying part of this all is if I continuously try to respawn while in my bed on fire, I end up respawning to the initial origin, a total of 6600 blocks away!

I wake up after dying in the Nether (when on fire like by lava or fire) in my bed in a Stronghold 1300X and -5300Y away from the initial origin, but I am on still fire. I click respawn, but I respawn in the same bed on fire (unable to activate things like buttons, beds, chests) and sometimes die immediately (or never wake up sometimes, though this is less repeatable). To avoid this, I have to quit to the main screen instead of immediately respawning, and I do not respawn on fire and die and arrive safely in the last respawn point. However, if I do not do that, I run into the prior issue, even if I pause and exit.
It seems to happen when I have poor cell signal or am unable to connect to the wifi without a username and password, though this is not proven. I thought maybe it had to do with a lot of open apps and the low battery mode, but this is not proven.

Here is a link showing that this is repeatable:
https://1drv.ms/v/s!AnRNw3cOQo1dgZ4KppxaemFITeSpsg
The steps are as repeatable as I described them:
1) Set spawn point in the bed in the library.
2) Die a few times in the Nether by fire.
3) Respawn immediately, not going to the title screen.
You’re welcome! Single player.
I fell from the void in the end and faced the dying repeatedly issue. I'm wondering if it is because my bed was next to a fence, so I moved it one block away. I will let you know if it happens again.
Another time, not sure if this is related, my character jumped onto the bed when re-spawning as if it wasn't done jumping from taking damage (by the ender dragon).
