It is quite possible that your pressure plate just has not enough time to retract, try making a command block teleport you further, for example 10 blocks instead of 7.
Try disabling a resource pack to a vanilla one, and see if your problem is still there, also try it in a brand new world.
And I am not a pro, but holding F3+C will crash your game, so that you can give the pros a log to work on!
I know that, but developers have the POWER to adjust the code to fit logic, I am a modder myself, and I think that such minor changes are easy and can in sum give a big red '+' to the game. =d

The problem is that when I build it, it works for about 5 blocks (on the 2nd - 3rd try) and increasing the distance results in a failure.

Have you tried it with a longer tunnel like 20 blocks or so?!? Because pressure plates don't get "unpressed" instantly, I suspect that the pressure plate has not enough time to retract before you run over it again.
I have tested it again and again and I can't find any problems with that, if you are to close, the pressure plate is pressed down, which is not a bug, and otherwise everything works as expected.
Eh, the 3rd coords are relative to THE COMMAND BLOCK/PLAYER, not to the structure that you selected!
Be sure that:
a) Your book is indeed in item slot 2! 2 not 1, since in programming the first thing has a number 0.
_b) Your book has a damage value of 1.
Also I don't remember, but try using book's ID instead of name.
