This bug affects all versions ever released, beginning at about MineCraft 1.2.
The day I discovered this bug, I was playing on Survival mode on [I think] Minecraft 1.2, and I finally acquired the resources needed to build two Iron Golems. I decided to build them in an NPC Village I found (there WEREN'T enough Villagers and houses for Iron Golems to spawn naturally), so as to help defend the Village. (For some reason, Iron Golems would just stand around and do nothing if I didn't create them nearby one or more Villagers.) At some point later, I exited my world so I could do something else.
Later, I came back to world to play some more on it. While killing mobs for loot (I did this close to the Village so that the Iron Golems could provide me additional defense), a Villager ran right in front of my line of sight, so that I wound up accidentally hitting him. Next thing I know, my Iron Golems end up attacking me, trying to kill me. |=(
This bug occurs also on Creative and Hardcore mode, as well as Survival mode.
How to Reproduce:
1) Build an Iron Golem in a Village (it can be either natural OR artificial, as I've also witnessed the bug occur in villages I built; preferably the Iron Golem should be built close to at least 1 Villager); if done correctly, the Iron Golem will proceed to behave like an Iron Golem that spawned naturally in the Village.
2) Exit the world, then return to it (length of time between exiting and reentering the world doesn't matter).
3) Do something that would provoke a naturally-spawned Iron Golem into attacking you (i.e.: Killing a Baby Villager); preferably nearby it.
4) The Iron Golem YOU (the Player) made should attempt to attack you.(Note: I've started to notice that, at some point before the 1.4 release (I can't remember exactly when), ONLY on Creative mode, the likelihood/frequency at which an Iron Golem(s) I made would try to attack me after being provoked by me, gradually began to lessen; by the time 12w49a was released, this bug would no longer occur [in Creative mode]. It still occurs as normal in Survival and Hardcore mode.)

I tried building one the correct way, in 12w50a and 1.4.6pre, and it didn't work.
...Oh yeah (sorry), I accidentally forgot to mention (somehow) that spawning a Wither doesn't work IN THE END.
While messing around on Creative Mode, I gave a Baby [Villager] Zombie some armor and a sword I enchanted, for it to use. Later, as I was going to do something else in the world I was playing on, I went back to that Baby Zombie to retrieve my stuff, so that it wouldn't despawn while carrying my stuff. But then, when I killed it, the Baby Zombie didn't drop ANYthing--let alone not dropping my stuff, it dropped no experience orbs, no rotten flesh, NOTHING!
What I expected to happen:
The Baby Zombie would [at the very least] drop the items I gave it.What actually happen:
The Baby Zombie dropped ABSOLUTELY NOTHING!Steps to Reproduce:
1.Find a Baby [Villager] Zombie that can pick up items.
2.Give the Baby Zombie a weapon and some armor. (The stuff I gave after I discovered this bug were: A Diamond Sword enchanted with Sharpness V, Looting III, Knockback II, and Fire Aspect II; a Diamond Helmet enchanted with Protection IV, Respiration III, and Aqua Affinity I; Diamond Boots enchanted with Fire Protection and Feather Falling IV; a Diamond Chestplate enchanted with Projectile Protection IV; and Diamond Leggings also enchanted with Projectile Protection IV.)

It's no longer possible to place the alternate double slab block (the block that uses the top/bottom texture of the stone slab on all six sides) as a result of this bug.... =(
I can confirm that this bug still exists as of 1.4.4.
Dinnerbone and Jeb said at an earlier point in time that they fixed this bug--the real thing is, though, that (I think) they merely reduced the frequency of it happening by 80-90%. (This was probably unintentional....)
A couple of times while testing this bug, I found that if I were to give a regular (adult) zombie armor and a sword, exit the world, reload it, and then kill the zombie, it wouldn't drop the items I gave it; it would only drop its normal loot. This discovery was completely accidental, but I can't seem to reproduce it anymore for some reason....
Shift-clicking items to remove them from your inventory doesn't work in Creative Mode if the Survival Inventory tab is opened/displayed at the same time.
I've noticed this with stairs and some slabs as well.
I think this should be made into a feature EXCLUSIVE to Creative Mode--items normally unstackable would stack in Creative Mode. This could help me save inventory space that I would rather use for other items.
I found another bug as a result of this enchantment book bug: If you enchant a Nerfed Golden Apple (the one made with the Gold Nuggets) in this manner, it doesn't a glow to it. (See "Glowless Apple" picture above for proof.)
Recently, I accidentally hit an Iron Golem in the head, while it was standing idly right next to a Creeper; but at the exact moment I did that, the Iron Golem attacked the Creeper. (Unfortunately, I couldn't get a screenshot in time....)
Through rigorous and extensive testing of this bug, I found that, under certain strenuous conditions—the kind that could lag MineCraft to the point of crashing/freezing indefinitely (i.e.: approx. 100 Villagers and Baby Villagers being chased by about 200+ item-wielding Zombies, Villager Zombies, and Baby Villager Zombies, being in turn chased by 6-8 Iron Golems, all under the influence of Extended Speed I and II, Extended Strength I and II, Extended Regeneration I, Extended Slowness I, and other similar potion effects)—the Iron Golem bug could still occur. (Unfortunately, I couldn't reproduce the bug in that same method a second time for the purpose of taking a screenshot of it.)
This still occurs in even the most recent versions (up to 1.4.6pre).
I also saw this on 1.4.7, on an SMP server! (I didn't think to take a picture of it, though. Sorry....) I've also seen this bug occur on Survival mode (namely, on that same server) AND without mob griefing disabled.
In some cases, where I would want my armor to last longer, I'd prefer a functional Unbreaking enchantment for armor over any Protection-type enchantment....
So Zombie Pigmen can't be friendly?
