In the screenshots I show two similar setups with 4 pistons all connected by redstone on blocks directly above the pistons, and a lever over each piston. Turning any lever on will extend all 4 pistons, but which pistons retract when it is turned off again is determined by which lever was used. The screenshots show the results after flipping the left-most lever on and off ( -after-1), then the second left-most ( -after-2), etc.

I attempted to reproduce by aiming at the center of a 2x2 wall, fired arrows clustered slightly to the right of the crosshairs.
I cannot reproduce this problem.
I'm having the same issue. I've attached the crash report after manually crashing the game.
I logged back in from a different minecraft installation on a different OS, had the same issue. I've attached the new manual crash log from windows 7.
I can provide the world save if needed.
