I have the same issue on XBox One. Most of the inventory gets reversed. Where it starts and how much is reversed seems random. The change is not specifically from acquiring new gear. If you sort then close and reopen the inventory the items will have moved. This does not happen everytime. If the inventory is sorted a section will be reversed. If left in this state the inventory will remain as is. If it is then sorted again a different section will be reversed. Hope this helps.

Since 1.8.0.0 this issue has gotten worse. Now instead of reversing a section, the inventory takes several sections, some reversed and some not, and splices them together. The inventory no longer remains in this state if left alone. Every time it is opened the process will repeat effectively shuffling all items like a deck of cards. This can even include new gear not yet examined by the player. Hopefully this helps. This bug was annoying before, now it makes inventory management effectively impossible.
My fiance is also having this issue. If I am P1 the cape appears in her inventory, but she can't equip it. If she is P1 the cape doesn't even diplay in either of our inventories. She can't equip it at all and I can only equip it if I'm P1. She has completed the 7 requisite missions once as P2 and again as P1.
Yes. This is still an issue on Xbox at least.
This also occurs on the Nether Fortress mission, not just in Ancient Hunts.
