When using the enchantment table on a vanilla 1.8 server in a test of 200 books, 48 iron pickaxes, 15 stone swords & 37 stone shovels over a range of different levels, 96% of enchantment where a single enchantment.
From what I understand this is extremely unlikely to statistically happen so I figure it has to be a bug of some sort.
So far only experienced this in a server environment, one running command blocks in the background and another running 100% vanilla (no commands).

Cannot confirm (have no numbers, but got in survival ca. half of the stuff with 3 enchantments)
What XP levels did you have while enchanting and did you always take the third option in the enchantment table ?
When testing I started with 30 XP on me and then kept enchanting until I was unable to enchant anymore with the levels I had, removed the surrounding book shelves to reduce the require levels and then continues this process until I was out of xp.
As for always taking the 3rd option, no I mixed it up between all 3 options.
Well, you get a high rate of 3 enchantment stuff only with 30 levels and max. bookshelves.
