MC-98213Mending not repairing main hand item
Diamond pick with mending, unbreaking III, fortune III and efficiency IV in main hand being used to break blocks will not repair and experience orbs go to experience level. If same pick is moved to off hand it is repaired.
I would expect an item with mending when held in the main hand to be mended.
With 4 items in inventory with mending enchantment, 2 armor pieces and 1 item in off hand and 1 in main hand all got mended except the main hand item being used to break coal, redstone and diamond blocks.
Steps to Reproduce:
1. enchant pick with mending
2. use pick in main had to break coal block
3. watch experience go to experience level instead of repairing pick

Mending not repairing main hand item
Cannot confirm [for 1.9-pre4], my pick repairs itself just fine. (Using a flatworld for testing and coal ore spawned with /fill)
Which item is repaired is random. What happens when you remove all other items enchanted with Mending?
Actually, I'm having the opposite. Mending only repairs my armor/offhand/mainhand items. (I filled my whole inventory with the diamond pick you described & diamond armor with mending, but none of the picks have been repaired.) I have a feeling my "problem" is Working As Intended, though.
Armor and off hand items were randomly repaired fully and never encountered any more damage with no repair to main hand pick. Next try was with main hand pick only item in inventory with mending, still no repair and all exp orbs going to experience level.
Next try with another pick with mending only enchantment and no other mending items in inventory. Used pick to create damage then tried on coal. No repair, all exp orbs went to experience level.
I still haven't been able to re-produce this, maybe add a world download?
It seems the issue is a corrupt world or related to pre-existing worlds. Started new world and mending does repair main hand item. Problem remains in existing world.

Is it possible it's caused by the item having " Repair too expensive " ?
I'm having the same issue with a bow with Infinity, Unbreaking III, Power IV, Mending.
Adding Mending required 33xp. equiped as either main hand or shield, xp just gets added to bar instead of repairing bow.
Bow is at approx 10% remaining.
Belay that, it both repairs AND gives me XP .
I am having this issue as well on Minecraft Xbox One. MEnding on my fishing rod is currently working and I do not have it on any of my armor, also tried removing armor. It simply does not mend my diamond pickaxe.
It's been over a year since the question was asked, please respond to the question or attach the files requests and keep the ticket updated.
