MCD-1602Softlock in Creepy Crypt
At the exit of the dungeon section in creepy crypt you can go through the exit door, while the barrier is still there causing you to softlock and forcing you to restart the mission as you cant exit the mission through the exit door because you don't have completed the quest. (And you can't go back into the dungeon area because the barrier is still there
