It's bugged. You have to do it all in one level as progress resets between levels. Get some gear that spawns food items and sit & get all 200 in one level instance and your trophy will pop. A lot easier if you can get a friend to help as you can spawn, eat, suicide and be revived with full charges and spawn again instantly. With an optimal setup & a 2nd person you can have it done in 10-15 minutes if you're sat in a quiet place by some lava.
