Hey, I've had the same issue. However, it happens in any level at any time.. I've done some looking around and people seem to say that it is if you have the "Surprise Gift" Enchant or "Food Reserves".. They say it happens when you click(consume) the item while your character is standing inside the item.
I haven't tested this.. But have had it CONSISTENTLY happening to me as I've been powering through levels meleeing like a mad man.
Only solution I've found is by using a controller rather than a mouse/keyboard. (which I hate using controller)
(PC) This bug is driving me mad. But I will say I do NOT have "Food Reserves", but I do have "Surprise Gift". I only get this while using my Mouse/Keyboard. If I swap to using my PS4 controller I don't get the bug. Extremely annoying. Please fix!
