Changed Description:
If you're experiencing a drop in FPS, even for a short amount of time, you may glitch through pistons if you're standing on them. Here's a build that might demonstrate this: https://www.youtube.com/watch?v=NZbJ-ldIgMQ
This glitch won't happen if you aren't lagging.
I also believe that lag may cause the pistons to behave abnormally, like not extending or extending at the wrong time.
1.9 is also laggy for me, even though older versions (excluding 1.9 snapshots) don't lag, as long as I'm not running big resource packs like Faithful. The main menu also lags.

If fixed, would this resolve singleplayer and menu lag?
I'm also going to mention that using VBO's, Vsync, render distance minimum, allocating RAM, OptiFine don't help. Do not mention getting new/upgrading PC, driver, graphics card, etc. as it is NOT possible.
Tried latest snapshot, menu still lags. I'll also mention that VBO's, Vsync, minimum render distance, OptiFine, allocating RAM don't work. Do not mention getting new/upgrading PC, graphics card, drivers, etc. as it is not possible.
OptiFine dosen't fix any lag issues. In fact, none of the suggestions the entire Minecraft community has made has actually worked. I'm pretty sure the only way to resolve this is for Mojang to rewrite some of the code. On the other hand, previous versions work fine, even without OptiFine (sometimes, I can even use max render distance without any problems).

One of them is the 16w15b, one is 1.9.2, one is 1.8.9. All were taken in the same superflat world.
Just question: does 1.9 have this new render system or something that requires new drivers>
I once saw this in a 1.9 snapshot. This also appears in 1.10. I believe that your mining speed has something to do with this.
I've attached one of the worlds this bug occurred in, however, it seems to appear consistently in all worlds.
Actually, they do disappear when you right click them, my bad.
