Lighting glitch, appeared when i was placing white stained glass. In some area it doesn't affect it.

When I stood on a pressure plate with some command blocks "attached" to it using testforblock, I was dissconnected from the server.

Textures of all the stair blocks are overlapping (facing: west/east, half: bottom, shape: inner_right/inner_right) or the textures are to big on the side (all directions).
Edit: The texture on the back of the stairs (when facing north/south) is sideways.

When I use this command in a repeating command block, the corrosponding block turns into a ghost block.

Problem solved: It has to do something with LogMeIn Hamachi and his IP. I removed the IP adress and it (at least) works again. I use this program to host my server. LogMeIn Hamachi get broken every time i start up Windows 10.

Also a question. Are duplicates bad, because it looks like from your comment that duplicates are a bad thing to have.

Just found out this is a duplicate, please remove this bug report.
Also affects Title screen: the players skin partially obscures the logo and completely obscures the yellow text

I think this is not really a bug, it's the game trying to save and unload the world. But I also think Mojang should add a way to hide this with something like a text saying: Saveing World.

Is it a modded version of Minecraft 1.9.4 or pure vanilla?
Try this
Short version: Windows + R > %appdata% > .minecraft > versions > delete the 1.9.4 folder > Open Minecraft in version 1.9.4
