Was using a recently made diamond sword enchanted with looting III, Smite IV, Knockback II. Used it a few times and it broke. Made another through creative on LAN (with smite V), switched back to survival, it broke again. Once with enderman then with a creeper.
Items currently held in hand disappear when any piece of armor you are wearing breaks

This must be what is happening! I reported the issue being as my sword randomly breaking, but if your post is true this explains it. Someone needs to confirm this and fix it.

This has been around forever, its something to do with the chunks loading and the background being the sky or something... if you align it, you can actually see the sun setting in a long tunnel underground.
This jsut happened to me and after checking other worlds that ARE found and my old back ups of my main world, i see that the only file that is missing is the level.dat_old File. This happened to me after i entered the nether, traveled a large distance and exited the nether, crashing the game. I have seen this bug for a while, but I have never seen it do something THIS destructive. If this isnt fixed soon there are going to be several single player worlds lost.
