hollowell, the string doesn't precisely state the same thing twice, but I understand where you're coming from. The first part of the sentence states that someone is threatening to harm themselves, and the second part states that someone is talking about harming themselves.
I personally feel that this string is currently a bit too wordy as it's quite repetitive. A better alternative in my opinion would be something along the lines of, "Someone is talking about or threatening to harm themselves in real life".

hollowell explains our initial reasoning for well, but I like these suggestions. As there is a separate category for "Imminent harm - Threat to harm others", I think the copy here should be as [Mod] Avoma suggests:
"Someone is talking about or threatening to harm themselves in real life."
