I have the Infinity enchantment on my bow, so I only have to have 1 arrow in my inventory.
I renamed this arrow so that it wouldn't stack with the arrows collected from killed skeletons.
But if this arrow is shot by a normal (unenchanted) bow, the arrow is picked up as a normal arrow instead of the renamed one.How to reproduce:

Baby Zombie on its own can't go through the trapdoor (covering a 1 block space) as the entity is 1 block high.
At my mob farm I noticed the Baby Zombies riding the Chickens can just go through the trapdoor as the chicken is just 0,7 blocks high. With the trapdoor (0,1875 blocks high) it adds up to 0,8875 so it's just below 1 block high.

I killed 3 patrols it gave me 3 Ominous Banners in total and they could be stacked.
It doesn't show the patterns until you place the banner, but then you can't stack that banner with the other 2 which haven't been placed. If you place these down, you can stack the 3 banners again.

I was actually just trying to fix the problem by Optimizing the world in 20w20b (in Edit.. menu) when I saw your comment.
I loaded it up in 20w22a with the log open and it didn't have any problems anymore

I also have this problem with my iron farm, my waterstream is 3 blocks wide
My temporary fix is by placing signs on the walls, so only the middle one can be used for spawning, the Iron Golem needs 2 block of space so it won't suffocate in the signs
