i am also having this issue. I haven't been able to figure out conditions exactly. that said, I feel that if there are large mobs (cow level is where it happens for me) and a mob drops an item that is when it goes haywire. first few times it seemed like grabbing item, which was not easy, solved it. last time i had issue it would not fix itself because item dropped into a hole. I tried jumping off cliff to respawn and I got trapped in same hole. I was technically still on map So I didn't die. I had character outline and could mouse over the potion but couldn't move.
