Changed Description:
When exiting from a minecart, the game seems to pick odd/bad spots to place the player. Even when the minecart has plenty of empty spots right next to it, the game will still pick a spot several blocks away (could be a directional glitch). When in The End, instead of picking from a few spots on a small platform near the cart, it placed me over the void (i.e. not on a block).
EDIT: even when I place a block where I fell into the void last time, the game decides to drop me somewhere else.

Seems to only appear when the "break" particles are rendered (i.e. when you break a tripwire).

I tried searching for "minecart exit", didn't see anything immediately. It was a bit of an issue for me in 13w42b, but still tolerable; not the case for 13w43a.
14w02c is affected as well - whenever I save and exit with a horse right next to me, and open the map up in MCEdit, there's no entity next to me. Indeed, when I log back in, the horse and its saddle/armor are gone, as if they weren't saved in the level on exit (I searched the world for the horse's UUID from a backup; nowhere to be found).
Based on what letters are affected ('e' is affected as well), I believe it's because it's trying to match the "minecraft:" prefix - indeed, all the letters you listed are in the word "minecraft". I'm guessing this was intentional, but maybe it's not.
Same goes for multiplayer

Did you reuse an old map name? Perhaps they're chunks left over from a previous save?
This actually seems to be an age-related issue - should I create a new issue with a more accurate title/description?
When I created a new SSP world, the banners swayed as expected. When I hopped on my several-month-old server, they did not. When I copied the DayTime and Time NBT tags from the server's level.dat to the SSP world's level.dat, the banners did not sway in that world. I'm guessing it's due to larger double-precision numbers aren't as accurate after the decimal.
Well to be fair, this was reported a day before
MC-63720, but if that one has better info, then it's fineLooks like you're just running out of memory, aka you should increase "-Xmx1G" to something like 2G or 4G.
