Edit-14w05b and 14w06b now tested and confirmed
When lava pools generate in a forest or taiga biome, a nearby tree's leaves can become stone.
How to recreate: Find a taiga or forest biome, and a lava pool. if there is a close enough tree, some of the leaves become stone, granite, andesite or diorite.
this could be fixed if the lava pools were generated after trees, or if leaves were excluded from being replaceable blocks for the pools of lava.
Edit: will provide seed + coordinates when another example is found, i accidentally deleted my world...

World is superflat, its hard to find a default world example. they do exist though.

still in 14w04b, and all official release versions.
it seems it happens often when an entity walks on stairs or slabs. it's common in villages and around houses with such blocks.
i understand that, but the fact that the Iron golem breaks the lead on its own nullifies the purpose of using it.
I know what happens with your cave entrance dissapearing. after you leave the game, you aren't giving the game enough time to save the worlds. after you pause, wait a few seconds to let the game save the world. I dont know about your skeleton, though.
confirmed in 1.7.4 and all later snapshots, extremely annoying and needs to be fixed.
it still happens to me in 06b
I attached the world save file above the screenshots, did you try that? maybe theres something wrong with the original file...
i was unable to reproduce in 07a too. I think it is fixed.
happens to me >50% of the time in 14w07a,even at y=15, i know because I lost diamonds that way. it is extremely annoying, hope it's fixed soon!
I've been testing this out, and it seems that this bug is caused by
MC-315. oh well.
