MC-15449Getting shot by Skeleton makes equipped item disappear sometimes.
The skeletons arrows seemed to make the the items I was holding disappear. An iron pickaxe I had equipped disappeared first, then a diamond sword. I was pushed into lava shortly after, so I cannot experiment more. It may be that I hit Q accidentally, but I am pretty sure I did not.

Something similar happened to me, while fighting mobs equipment that you are holding seems to disappear.
This is actually a duplicate of
MC-15416.You're right. My armor must have broken at that moment, I know it was nearly depleted. Glad to see that it is fixed.
I had that bug twice.
1st time my diamond sword (echanted) disappear when i was fighting skeleton.
Second time that happen to my bow (enchanted too).
