I think I know the problem
Sometimes minecraft doesn't delete the entire folder when deleting a world. Then if you generate a world with the same name, the chunks are loaded from the previous world and generate the rest as the actual world. I suggest that you delete the world, and check your saves. If the file is still there, delete it.

Have you tried deleting these incorrectly generated chunks via some third-party program such as MCedit. From the results of this test you could determine whether this is a slight flaw with flatland generation or the game neglecting to read the NBT tags of the save before generating these chunks and therefore generating them as it would with a normal world. This also seams to be the same issue as MC-204.
this has happened to me too like 4 times i took pics. look up superflat and there should be a error report by nathan welch (me)
