Ores with Spawn Size '1' or '2' don't appear in the Customized world
The bug
Making the Spawn Size value for any ore/stone variant (e.g. diorite or gold) a value of 1, or 2 causes zero of that thing to appear in the world.
How to reproduce
Change all the Spawn Size values to 1 in the Customized GUI, create a world, and then investigate. A fill-replace of stone->air is a good way to have a quick look.
At a value of 3, it starts to appear in clumps of 1 or 2, and at higher values, it behaves more like one would expect.

Configuration oddities of 'Customized' world presets
The 'Customized' world type allows you to twiddle lots of knobs with world generation, but there are some odd behaviors. Here are a couple odd thingsI have noticed.
First, the default 'dungeon chance' appears in the GUI slider as '7', but in the default preset it has the value '8'. Why the discrepancy?
Second, making the spawn 'Size' value for any ore/stone variant (e.g. diorite or gold) a value of '1' or '2' causes ZERO of that thing to appear in the world. (Try it, change all the 'Spawn Size' values to '1' in the Customized GUI, create a world, and then investigate. (a fill-replace of stone->air is a good way to have a quick look)) 'At a value of '3', it starts to appear in clumps of 1 or 2, and at higher values, it behaves more like you'd expect. Why do low values not work properly?
This has probably been a bug for a long time, but I only tested it in the 1.9 snapshots.
