We had multiple instances of separated areas generating that cannot be reached. However, they're still shown on the map. See attached screenshots.

Still occurs in 1.3.2.0, and shows up in a few more levels. Found this in Soggy Swamp today.

I am encountering this too. On Creeper Woods I often get so I can't get one chest that ends up in unreachable area.
