It looks like the _JAVA_OPTIONS setting might be causing problems. To resolve, you need to remove this setting:
- Press the Windows key (between Ctrl and Alt) and the Pause/Break key (at the top of the keyboard, above the arrow keys) at the same time.
- A window should popup, click on 'Advanced system settings'.
- Click on 'Environment Variables'.
- Delete the _JAVA_OPTIONS entry.

Have managed to replicate twice, both occurred with a fresh start (no .minecraft present). The 'Play' and 'Log out' buttons are greyed out and the profile '(Default)' is selected. Development console: https://gist.github.com/stupid3700/7ea6b7281326c994282b
This workaround no longer works as of launcher 1.4.7.
I've found this workaround to work:Click 'New profile' in the bottom-left, give it a name and click 'Save profile'. From the drop-down box in the bottom-left, select the newly created profile and a login screen should appear. It sounds like you have touchscreen mode on.
Go to Options > Controls and set 'Touchscreen Mode' to off.
If you have an AMD/ATI graphics card, remove the beta drivers if installed and install the stable 13.12 release.
If you don't, or that doesn't help, go to Options > Video Settings and slide Mipmap Levels to the left until it's at 0.
To resolve this issue:
On the Start menu, search 'cmd'. Right-click the 'cmd' result that appears and select 'Run as administrator', accepting any UAC prompts.
In the command prompt window, type:
and press Enter.

The Intel 4 Series (and older) video cards are unable to play 1.7.4 and future versions of Minecraft. You will need to buy a new video card or computer to play new versions.
For now, you can still play 1.7.2 or older versions of the game. To change your game to a different version, start the launcher. Select Edit Profile. In the Use Version dropdown menu, select the game version you want. Then click Save Profile.

I'm afraid I personally can't reproduce any of the issues you've described.
The world for which you have provided a download works fine for me, and the Nether loads as well.
You can try resetting your Nether world by going into that world's folder and removing the DIM-1 folder.
If you can reproduce the issue you're experiencing (including the issue with the Nether), and then afterwards, attach the 'latest.log' file (located in .minecraft\logs, and it might just be called latest), it may provide other information about the issue.
This suggests an issue relating to your graphics drivers. You should refer to the technical support resources for further assistance with this issue.
If you could include the entire contents of your 'Launcher Log' tab (right-click the text and select Copy All Text) in the report, your issue may be more accurately identified.
This looks like it could be an issue with your graphics drivers. As this is most likely an issue on your end, and not a bug with the game, you should refer to the technical support resources for assistance with this issue.
The crafting recipe for fences changed in 1.8 and no longer uses 6 sticks, instead using 4 wooden planks and 2 sticks, where the two sticks are in the middle 2 boxes, with 2 planks in the 2 boxes to the left and 2 planks in the 2 boxes to right of the sticks.
For an image showing the crafting recipe, see the fence page on the Minecraft Wiki.
In order for your issue to be identified, you'll need to include your Game Output log in this issue. To copy the text on the Game Output tab, right-click on the text and select 'Copy All Text'.
In the event the Game Output tab is completely blank, include your Launcher Log tab contents instead. To copy the text on the Launcher Log tab, right-click on the text in the tab and selecting 'Copy All Text'.
This is a technical support issue, and is probably caused by security or antivirus software.
For assistance, please refer to the technical support resources.
As was said previously, your issue is the same as
MCL-774, as you have an exclamation mark in your Windows username. Please see that issue for instructions on resolving the issue.
In order to identify your issue, you'll need to attach the contents of your Launcher Log. On the Launcher Log tab, right-click the text and select 'Copy All Text', then include the contents in this report.
Please attach the log as a plain text file as in its current format it requires specific software to be viewed.
Paste your Launcher Log contents into TextEdit, select the 'Format' menu and choose 'Make Plain Text'. You should then be able to save it as a plain text file and attach it.

If the latter, are you able to reproduce the issue again yourself?
