MC-83319A sword placed in off-hand cannot be used/has no effect
Off-hand weapons are unusable.
Placing a block in right hand and sword in off-hand allows you to block with off-hand, but attacks with block.
Having a sword in both hands leaves the off-hand useless when it should be usable.

How is that intended? You should be able to use the weapon in your off-hand.

You cannot use weapons in your off-hand. That's how it is supposed to be. http://imgur.com/a/MuCZF
But the sword in the main hand should not have a "use" if there is something in the off-hand. Going to create that report now. This report is indeed invalid.
