Parrots lose their Name Tag upon dismounting a player's shoulder
I went into Creative Mode on Minecraft version 1.12-pre5, and decided to mess around with the new Parrot mob. I tamed several while still in Creative Mode, and decided to name a few of them with name tags just for the heck of it. I took Name tags and an Anvil out of the Creative menu, placed the anvil, and proceeded to use it on one of the Name Tags. I named it "Cracker Jack", because one of the Parrots was a Cockatiel variant and made me think of a cockatiel that I had in a classroom in high school by that name.
I applied the Name Tag to the Parrot in question. The Parrot then perched on my shoulder, just as they all do sometimes when tamed, but when it dismounted upon me jumping in place (something I knew would happen because it was mentioned on the wiki), I noticed that he no longer had a name above his head when I looked at him up close. I tried naming him again, and after he perched on my shoulder and dismounted again, he had once again lost his name tag. I tried this with different names on different parrots, and the same happened every time: when the parrot perched on my shoulder and dismounted any amount of time later, it would no longer display a name above its head when looked at from within a couple blocks distance as they should have. I tried this again in Survival Mode (using name tags and the anvil that I gave myself in Creative Mode), and got the same result.
So, step by step guide on how to reproduce this bug:
1. You must have an Anvil placed on the ground and a Name Tag in your inventory.
2. Name the Name Tag in the Anvil (what the name is does not seem to matter, according to my tests).
3. Apply the name tag to a tamed Parrot.
4. Allow the Parrot to perch on your shoulder. I looked at it on my Player model in my inventory just to confirm that it was on my shoulder, but that might not matter as long as it perches on your shoulder.
5. Perform an action that causes the Parrot to dismount your shoulder (I jumped once in place to do it).
6. Once the Parrot is off of your shoulder, look at it up close to see if it displays its name or not. If it does not, the bug I am reporting has been successfully reproduced.Environment

@Josh Riley I reduced the description of the report a little bit, I hope you are fine with that.
@Marcono1234 Yes, that's fine. I was a bit overly descriptive before.
