Do you have spawned the phantom with NoAI=1 ?
Did you disable the vanilla data pack ?
It is also possible that we configured the problem, and then wait 2 days after the configuration to see it again.
It may be that the CPU is too low.

It only happens after you play for a while
It seems to happen to me when I play on a multiplayer server but when I play on LAN, it animates fine.
Can confirm that this happens on a dedicated multiplayer server. Just tested it with 1.13 and 18w30b.
I seem to encounter this issue on servers what were made prior to 18w19a
Maybe a world age issue like

I can confirm this is still happening in 1.18.1 and even 22w05a, in a multiplayer server I'm on Phantoms do not flap their wings

This should be reopened, the requested info was provided by another user (not reporter), 2 years after resolution, so the bot didn't reopen it.
