Has this issue been occurring for anyone since the latest update?
This bug has happened to me on Cacti Canyon on My Nintendo Switch. Game Version: Latest Update: Creeping Winter.

Is this still an issue in the latest version? And if yes, then what is the repro rate (how many times did it happen over the course of how many playthroughs)? If it does occur in the latest version, then can you please add it to the list of affected versions (or mention it if you are not the reporter)?

I was able to reproduce this just recently. When I killed the skeleton guard in the desert temple, the key golem "died" with the skeleton guard instead of escaping.

This issue occurred to me in Cacti Canyon; the key did not even spawn, or it spawned underground in a place I could not even detect.
Alright, so this issue is confirmed everywhere, right? Colossal ramparts, the nether, cacti canyon, Desert temple.... etc;
Happened to me in an ancient hunt.
This happens in lower temple, when a mob gets the key, and then is killed in return, the key is often dead with it. This will make it disappear entirely and ruin the game. Looks like it happens in other places too.
Just experienced this during a daily mission in Highblock Halls. Good to know I wasn't just going crazy.
