So, either my Protection IV villager somehow teleported through the wall, or it despawned because of this bug.

Workaround (updated Dec. 5, 2020)
Since some cases of persistent mobs despawning can be traced to them crossing chunk borders between saves, one way to guard against randomly losing mobs is to prevent them from crossing chunk borders. However, this is not a guarantee or a fix. Positioning mobs away from chunk borders does not mean you will never lose a persistent mob again. Many cases of mobs randomly despawning remain unexplained by the tracker staff and player community.
Does putting them in a boat cause them to despawn? I've tried this, but they seem to be always there.
Please please please please upvote the issue if you experience this problem rather than comment if you do not have new information to add.

It also happens in local multiplayer. The torment quiver is locked on cooldown.
The hat is not in the character creator anymore.
I used an updraft tome on three windcallers at the same time, yet still have not gotten the "Taste of their Own Medicine" achievement. I also defeated the tempest golem with a bee from a bee stinger, a llama, a wolf, and an iron golem. I was also wearing a cave crawler. But I did not get the Herd Mentality achievement. I played on apoc+ 17. It was on local multiplayer on Xbox one, if that helps.
I also wanted to add, sometimes the achievements are delayed. I finished colossal ramparts, but got the Rampart Rampage about a few days later. As for the achievements I mentioned above, I still have not got those. The "Taming the Storm" achievement is still not unlocked.
I saw someone use a fishing rod to get the loot...? But this may only work on PC. I play on XBox one and still cannot access the chest
Cross-save for Minecraft dungeons has not been released yet.

Does not work for shulker boxes either. Tested in both survival and creative. I was trying to get the organizational wizard achievement by naming a shulker box, but I could not rename it. However, tools and armor can still be renamed.
happened to me too. Local split screen multiplayer.
I fed an ocelot some fish, went to look for another ocelot, but when I returned, my original ocelot was gone. I also tamed a bunch of parrots in the jungle. After making them sit, I ventured into the nearest jungle temple. When I came out, they were all gone. Playing on Xbox one, but not on realms. Split screen multiplayer. I wonder if this is a different bug since everyone is reporting this on a realms server??
Thanks @MD, I was curious. I do hope this bug gets fixed. Valuable villagers despawn randomly, too. I also saddled a pig, but it was gone after I went to the nether and back.

We stopped playing Minecraft for about a year, and then when we returned to play, the world never opened. Every time we clicked on that world, it would attempt to open, but it always crashed. After months, we tried opening the world again, and it finally worked. However, something was wrong. Terribly wrong. You can see the info_update block among the weird structures that randomly appeared. We haven't even gone to the end yet, but there are shulker boxes everywhere! Random items are scattered across the world and tons of blocks are stacked. It wasn't shown in the video, but missing texture blocks were also in the "buildings."
@Mew and Minami, you could continue your project, since this bug isn't always that common. Try staying near your villagers and avoid going to the nether. This isn't a guaranteed workaround, but maybe you can look closely and see if your villagers are despawning even when you are right next to them.
This issue occurred to me in Cacti Canyon; the key did not even spawn, or it spawned underground in a place I could not even detect.
Have you tried rolling or using a light feather to get out of that place?

MCD-4406This never occured to me, maybe I have to play more ancient hunts to come across this issue. Was this singleplayer or multiplayer?
Still an issue in 1.8.1.0. Its with almost every weapon as well.
Does placing down a scatter mines help? Maybe if you take damage, you can get out..? Also, this is just a random suggestion. It might not work. ¯_(ツ)_/¯
Alright, so this issue is confirmed everywhere, right? Colossal ramparts, the nether, cacti canyon, Desert temple.... etc;
I realized that you can use the character creator as player 2 and create your skin all over again, but this is is a waste of time since it will be deleted after you save and quit the game.
if you play in an online game, you can also use the teleport button to teleport to another player outside the "death trap or the stuck trap"... right? Or does the teleport button not work?
@Alex Groot, so this means this bug is not only caused by mobs crossing chunk borders. Did this ever happen before or was your horses despawning recent?
Also, I was looking at some older comments and I just wanted to repeat some important info to anyone else saying what happened to them. It was said that mobs that move a lot are most likely to cross chunk borders, which is why they despawn the most. I'm talking villagers, minecarts, and bees. This is also why more stationary entities, like armor stands, don't despawn much from this bug.

This happened to me on XBOX one S. It was just a small bug before 1.16.210, but then it became a major issue after the update. Before, restarting Minecraft would fix the issue, but now the 2nd player on split screen has lost all progress permanently. I have lost all my netherite tools and armor because of this bug. I know that my progress was reset and I did not just die, because if I open my ender chest, nothing is there! Please, Mojang, fix this bug.

@fp, there is no real solution to this bug, but it would help if you prevented your horses and villagers from crossing chunk borders by enclosing them in a fence or pen that is only in one chunk (16x16 blocks). You could also avoid going to the nether, because that also causes this problem. This won't guarantee your mobs not despawning, but its the only thing you can really do to help.
