Players in sometimes die immediately after entering a level
Sometimes, one or multiple players are immediately dead after having entered a level and need to be revived by another player.
To me, this only sometimes happened to the friends I played with in an online game, however it seems to happen in a singleplayer offline game as well (see MCD-175).
