Positions of entities become offset to clients in SMP
In an adventure map in SMP, I was experiencing strange issues where the locations of other players seemed to be offset. This would occur mainly in cramped places (on the map in question, in the dungeon at the top of the mountain).
I would notice players passing through walls, and sometimes floating or "swimming" in the ground. At some times, the offset seemed to be so bad that players would be rendered several blocks away from their true locations.

EDIT: Since then, I have realized this effect can be reproduced reliably on minecart entities. Steps to reproduce:
- Run your own server
- Set up a minecart rail loop with powered rails and a two block drop
- Add a minecart, and make sure it goes around the loop
- Observe that, after some time (this seems random), the minecart gets misaligned: although it tracks the same path, it may appear floating or offset by several blocks.
In my experience, lag on my system makes this effect more likely, as does moving around.

Sethbling may or may not be the server host; I'm sure Marc Watson of Mojang can clarify (he was present).
EDIT: Since then, I have realized this effect can be reproduced reliably on minecart entities. Steps to reproduce:
- Run your own server
- Set up a minecart rail loop with powered rails and a two block drop
- Add a minecart, and make sure it goes around the loop
- Observe that, after some time (this seems random), the minecart gets misaligned: although it tracks the same path, it may appear floating or offset by several blocks.
In my experience, lag on my system makes this effect more likely, as does moving around.

I recently made a bugfix mod for glitching mobs and even though I couldn't reliably reproduce this issue in 1.6.2, I think there's a good chance this should be covered as well: https://github.com/taurose/Unglitch (4).
In the tracker code there's a flag that determines whether an entity has moved far enough to send a position update. Several lines later, if the flag is true, the tracker stores the entity's location that is supposedly sent to the players for later updates. However, it seems that over time Mojang added several conditions to circumvent that flag and send or not send position updates regardless, which causes the "last sent" locations to be off. And once that happened, the clients receive false position updates and the entity appears offset.
Luckily, this can be fixed quite easily with a few lines once you know the problem.
If that's the source of the issue, this would be related to MC-342.
Multiplayer / Integrated servers have very poor performace and cannot keep up even with one player on a high-end PCs / Servers / Machines
Description
Vanilla server platforms (integrated and external) are very poorly optimized and can barely run smoothly, even on a high-end hardware with higher amounts of memory allocated.
Typical, small multiplayer server drops to critically low TPS and can crash / be forcibly shut down by the watchdog (if enabled) with just a few players normally generating new chunks (in survival gamemode).
A server, using the "Default GUI" is almost impossible to run stable. When launching it via command line and allocating sufficient amount of RAM, the performance is a little bit better (and the server is not running out of memory), but the playing experience is still very rough and there is quite a lot of rubber-banding, freezes, lag spikes, etc.
How to reproduce the issues & Testing methods
Singleplayer (Integrated Server)
- Launch the game with plenty of memory allocated (4GB is usually more than enough)
- Enter into a world or create a new one
- If creating a new world, initially there are extreme framerate drops (probably client-side issue), the integrated server is rubber-banding and skipping ticks
- Start exploring the terrain around and perform the usual things (generating / loading new chunks, dropping item entities, etc.) - There will be noticable stuttering, lag and rubber-banding when punching mobs or making redstone circuits, for example
- If the player is in creative mode (or spectator mode) then everything becomes much worse
If you are playing in a survival world for long enough (assuming there are animal pens / farms, some mob grinders, chest rooms with item frames and similar) the world will run much slower and this can cause issues with redstone circuits, influence on the gameplay and so on.
Multiplayer (Vanilla platform, without any mods / tweaks / changes)
- Start a server on a relatively powerful hardware with sufficient amount of memory allocated (4GB again, would be probably more than enough)
- Join the server, along with a few other players (2-4 players at the same time is normal, 5+ will be a bit heavier)
- Start playing as usual (exploring around, interacting with entities, mobs, items) - There will be clearly noticable rubber-banding and lag spikes, which usually result in stuttering entities, desyncing issues between the clients and the server and other
If some of the players start to generate new chunks on creative mode, the server will most likely crash.
Testing methods & RAM allocation
Tested and experimented with many combinations of JVM heap sizes (amount of memory allocated), using the default, "recommended" start-up command line:

Usual activities during the tests:
- Exploring the terrain around in survival mode, creative mode (for a while), spectator mode (just for the experiments, not expecting actual results)
- Interacting with mobs, entities, armor stands, droppping and picking up item entities
- Placing water and lava (creates many block updates)
- Small, medium and advanced redstone circuits (light, piston, hopper updates)
- Small, medium and large mob farms / grinders
- Updating gravity-affected blocks (sand, gravel, concrete powder)
- Entering different dimensions and teleporting items, mobs, entities through portals
- Transfering small and large amounts of items through hoppers
- Loading previously generated and alredy saved chunks
The RAM & CPU usage (especially) was abnormally high for very small and short activities or sometimes none at all. Not only chunk generation is causing the more serious issues (but mainly it is).
Conclusion
The main reason for most of the issues is not exactly the TPS drop / loss / poor server performance, but client-side ticking, updates and synchronization.
Other games (clients) handle the updates, coming from the server in different way.
If the server can't keep up with the consistend update rate / tick rate, then the client should enable some sort of "interpolation" and increase slightly the delay between the changes (updates).
The Minecraft client runs at hardcoded 20 TPS and waits for the next tick from the server. If it is synchronized (matches with the incoming client update / tick) then it runs normally.
But even if the server drops to 19.99 TPS - There is desynchronization once in a while and this can create issues (mainly client-side).
Overall, the server performance should be definetly optimized more and some things can be tweaked.

Steps to reproduce
- Run a minecart in a loop (see the screenshot below)
- Teleport 100 blocks away and wait a few seconds (/tp ~100 ~ ~)
- Teleport back (/tp ~-100 ~ ~)
The minecart appears to be travelling at an offset from the track. It might take a few tries. Make sure the chunks don't unload when you teleport away.
Code analysis
When a player starts to see an entity, the server sends the entity's current position to the client. But an entity also has another, internal position used for sending and calculating relative motion updates. This position doesn't get sent to the client with the spawning packets, so every consecutive relative motion packet is incorrectly applied. This continues until the next absolute position sync.
A proposed fix is to include the internal encoded position (ServerEntity.xp/yp/zp) in entity spawning packets and set it on the client (Entity.xp/yp/zp).
This only affects entities which are moving when they come into the player's tracking range and have already existed on the server side for some time.
Also, looking at ServerEntity.sendChanges(), the position is supposed to resync every, what appears to be, 20s (400t) but the timer which tracks that gets incremented only every updateInterval ticks (e.g. arrows get resynced every 400s). I'm not sure if that's intended.
This bug is related to MC-342, MC-2783 and probably many other similar issues.
Trivia:
This is a very old bug. I was able to reproduce it on alpha 1.1.2_01, one of the first multiplayer versions.
Oh, sorry, I had forgotten about this, as it's been a while since I've dealt with vanilla servers (I will note that I haven't experienced it in Bukkit whatsoever).
However, I recently saw Sethbling livestreaming on a vanilla 1.4.6 server: http://www.twitch.tv/sethbling/b/355261121
At 1:38:10, you can clearly observe that, after a series of teleports, players appear in the wrong positions for Sethbling (including the blaze-suit player sinking into the ground at 1:38:30, and the flying cactus-suit player just before that). Sethbling and other players were also complaining about "invisible" players, and frequent relogs were needed to correct the issue.
So, can this be reopened?

I've managed to get this now in 1.5 on minecarts, so it's not limited to players. Which is pretty bad, to be honest, but at least it makes it easier to test.
So far I've mostly managed to notice this with the player's not appearing to others after doing teleports. Been there for ages, as far as from beta 1.8. Players mostly appear back when you drop down a block below. Disappearing part seems to have something to do with the entitytracker, in MCP it looked like that player was not in the server's entity tracker after doing a teleport and player positions where therefore not syncronized to other players (until the player dropped a block below and then was added back into the entity tracker).
I found a video where the bug is extensively demonstrated and tested . Also, note at the end the client-side offset arrows. Considering this bug pertains to entity offset in SMP, it may be related, but probably isn't. But all of the teleportation bugs are textbook demonstrations of this bug.

Can experience this with Minecarts after a while in 15w51b
16W06A Player falls onto active redstone then sinks into block. Thus far cannot reproduce purposely altho occurred several times. Possibly related to early Elytra flight.
Is this still an issue in 1.10.2?
I personally haven't reproduced this in 1.10.2 but I did ask other players on a server I play on and they had. So it still seems to be present.
Is this still a problem in Minecraft 1.11?
I haven't reproduced it, but relative movement still is used, so it can hypothetically happen. I haven't seen it actually happen though, so if anyone has seen it, that would be useful information.
Does the way to reproduce in the description still work?
Shown to still exist in this video (at 1:15:36), which I'd say is sufficient evidence that this still exists (even though they're using optifine) when combined with the lack of any protocol changes regarding entity motion. It's not exactly the same setup as in the description, but it is a card going some distance and then getting desync'd.
I've tried to reproduce the issue by having a setup where a villager in a cart goes to chunks that are constantly loaded and unloaded, but I am unable reproduce the issue. Does anyone have a reliable way of reproducing this issue? If not, I will close this within a few weeks.
The video provided doesn't really tell us much about the server itself, it is possible the server had modificiations installed.
It also happened in Etho's LP, some time in episode 518... but I don't recall exactly when. It's worth noting that that's a singleplayer vanilla world, and again it's a minecart. But it doesn't help too much with reproducing :/
