Maybe it's possible a skeleton dropped a head when it died, and that zombie picked it up and equipped it (wore it as a helmet)?
Can regular skeletons drop their heads yet ?

I guess it could've been a zombie wearing a skeleton head, but I thought those drops were supposed to be rare. I've seen three more of them...
Is it possible it's just a texture pack issue? Also, is this Minecraft install patched with Optifine or some other mod that provides HD textures and/or random mobs?
I'm actually quite sure that this is a mod which adds random mobs. The skeleton head must be one of the options.
