Hostile Mobs spawn in Peaceful / broken difficulty setting in Realms
This is at least happening on Realms.
Hostile Mobs spawn when in Peaceful mode, switching to Easy/Normal/Hard and then back to Peaceful also doesn't despawn hostile Mobs.

Edit: I download my Realm today and noticed that the downloaded copy had difficulty set to "normal" (The Realm was running on peaceful the whole time). Switching the downloaded copy to peaceful and uploading it to the Realm solves this issue, until you switch to another difficulty again.

I can confirm that this is still happening in 1.4 on iOS. Really hope that they fix this soon because now downloading the world is bugged and I cant turn off monsters.

Same issue. Switched our realm to Normal then back to Peceful, but game still spawns mobs. And still get hungry, etc.
I just noticed a portion of this. I noticed that spiders would not despawn when switching realms to peaceful. I was switching back and forth between peaceful and normal to try to clear the mob cap. The spiders in the trees around me were not despawning.
I have not extensively tested, but toggling out of and back to Peaceful from the main menu (versus in-game) seemed to restore my realm to Peaceful. At the time, there were no players playing in the realm (in case that matters).
Happening in our Win10 Minecraft realm also.
I saw an enderman in your video, and a comment above says that spiders don't despawn in Peaceful. In Java, endermen and spiders are classed as neutral rather than hostile mobs. The spawn logic in Bedrock doesn't use quite the same classifications, but it makes me wonder whether these two are treated differently from other mobs and being allowed to spawn in Peaceful for that reason. So, do unambiguously hostile mobs (creepers, zombies, skeletons, witches) spawn in Peaceful as well, or is it only endermen and spiders? If it's only the two, that might help a developer find the problem more easily.
(I also saw the XP and rotten flesh at the start of your video, but I have no way of knowing whether it spawned in Peaceful. It might have been killed while in Normal and the drops were still around from then.)

Auldrick, the issue seems to be that in-game, changing the difficulty doesn't always actually change the difficulty (even though the menu reflects the change). Because yes, not only do all mobs still exist/spawn (not just the ones java considers neutral), you also still get hungry and can eat food. Where if the game was actually peaceful, the hunger bar stays full and eating isn't allowed.
I have better luck using the main menu to change the difficulty instead of changing it when I'm in the realm playing.
However, this is not the correct solution, before the last 2 update, this went on without such an effort to operate. Therefore, this bug should necessarily be eliminated.
Now that I understand it's a malfunction while setting Difficulty on the Realms servers, I'm sure it will be fixed the way you expect. It's quite common for people to offer workarounds in comments, but we don't regard them as fixes.
Still affects 1.2.13 realms on Windows 10.
Changing the difficulty to peaceful does not do anything, e.g. mobs don't disappear. Also, when going into the menu to change the difficulty "normal" shows briefly before changing to "peaceful".
Been over two months now. Anyone know if there are plans to fix this? It's rather curious that something as fundamental as the functionality of a menu item doesn't work. Note: I'm running 1.4 on an iPad Pro.
And can this bug be linked with the bug where the realm owner can't change player permissions any more? They were introduced at the same time. Both deal with menu items not working on realms. Probably related.
I'm having the same problem on the bedrock edition!
I’m still having this issue on bedrock as well. Can we please get a fix to this? It’s crazy that we pay for realms that don’t even work correctly when this has been an issue for over six months now.
Don't really have anything new to add, just letting you know I am having the same issue.
Everyone please vote for this. They just released phase two of the update aquatic and still haven’t fixed this issue.
I agree that this is still happening on a v1.5 realm.
I can also confirm that this is still happening. I am playing on v1.5 for X1X, Win10 and Android. I have had the issue of trying to change from Easy to Peaceful, to no avail. The game will say that it is in the desired difficulty, but does not actually change. The only thing is that recently, it would seem you have to have everyone out of the realm for about 2 minutes or so, and change it from there. That is what seems to have worked for myself.
I confirm this is still happening on Xbox One, Windows 10, and MCPE for iOS on realms. I have been having to download the world, switch difficulty, and re-upload the world to the realm.
I saw they fixed the issue with inventories not reflecting a world that was downloaded from a realm. How have they not fixed this issue as well, or even before this issue?! This realm difficulty issue has been a problem for much longer, I seriously hope they fix this soon.

This still happens in windows 10. The realm "remembers" my choice for it to be peaceful though, so if I leave the realm and a while later come back, it has updated to peaceful again. This is probably because the server restarts or something.

Marked this one as a duplicate, even though this one was earlier than the issue it duplicates. This since the other issue was the first one we got in the Realms project.
My realms is also not updating difficulty. When I change it to hard mode it stays easy when I go back in even after closing and reopening Minecraft.
