Artifacts don't reload
I didn't manage to reproduce it, but i've got a bug when i use an artefact, the cooldown was stuck and i was unable to use it again until the end of the mission. it was in Offline mod solo and i'm playing with an Xbox One controller. it's hapenned on the Mushroom, bind on the Y button. my 2 other artefact was still working fine.

Possibly the same issue as MCD-638.
This happened to my co-op while we were fighting the Nameless One in Adventure mode, her Flaming Quiver’s cooldown didn’t work. I will upvote this because I don’t want it to happen again.
I am using Flaming Quiver, Torment Quiver, and Firework. Sometimes they just stuck after cooldown and I had to unequip and reequip them to be able to use. I am using armor with "final shout", sometimes it happens after "final shout" activated.
Same problem: on boots,
final shout on armour,
after dying its work again
could not reproduce

this happened to me on windows 10, 1.1.1.0. i was using a torment quiver, and even though i had enough souls and the cooldown was over, i was not able to use them.
Same here. Firework rockets keep going gray after a Final Shout is used if the Firework Rockets are already in cooldown once they finish cooling down.
To replicate:
Activate the firework rockets.
Fire them.
Go under 25% health to activate Final Shout while the rockets are in cooldown
Fire off the new set, see it is back in cooldown.
When the cooldown recovers, the rockets remain gray until unequipped, then re-equipped or until you lose a life or exit the level.
I have had the same thing, Torment Quivers, Fire arrows. they wont come out of "cool down". after use. I had a full soul bar and could not trigger the Torment. had almost 500 arrows and could not use any of them. I can change them out and they are usable again.
I can confirm this bug as well for both firework rockets and the flaming quiver. It's really annoying because you have to un-equip and re-equip to clear the cooldown, or they will reload automatically when forced by the "Final Shout" enchantment.
had this with firework rockets. Was running 3 of them and switched one out, but another was locked in a cooldown that never happened. ps4 pro.
I have seen this with the Fireworks rockets as well. I do not use Final Shout on my armor, so that does not appear to be part of the problem. Thought it might be the bow enchant Infinity. but recently been using a bow without this enchant and still getting the bug, although it is less frequent.
My quick fix workaround is to remove the Fireworks rocket from the action bar, then put back on.
I have the same issue. My mushroom and rocket artefact stop working after a while which kills my build entirely. They stay locked and I can't use or remove them. I think it has something to do with cool down reduction. On my robe as an additional cd reduction and I have it on my gauntlets.

got same thing. used mushroom and spammed other artifacts, attack speed gets really fast with rampaging on weapon, mushroom cooldown went off and couldnt use it anymore in the same level, it got locked. once i left the level it started working again.
Have this issue with flaming arrows, but after the recent update, there is a workaround: I can open inventory and swap the non-functioning artifact with another active artifact, then swap it back immediately and close inventory. This seems to reset the status.
Had this issue with soul healer, torment quiver and lightning staff
I have the same Issue with Tormented Quiver, it happens if I 1. have the quiver active, 2. activate Final Shout on my Phantom Armor and 3. use up the quiver afterwards.
It happens in any of the 3 Slots, I can reproduce it 100% of the time.
I also have Soul Healer and Mushroom, both work normally.
Armor Stats:
Pwr lvl 51 - Final Shout lvl 3, Swiftfooted lvl2 - Common
Quiver Stats:
Pwr lvl 47 - Rare
This does never happen without the Final Shout enchantment.
happens all the time to me with the firework arrow

This happened to me a few times on 1.6.0.0 Win10. But last time it happened I couldn't even remove the artifact from the inventory, but I could do that before and thath would fix it.
It also happens in local multiplayer. The torment quiver is locked on cooldown.

This is still an issue for me in the latest version. PC, build 1.9.3.0_6386045. Happened once or twice in a sea level, then three times just now in Overgrown Temple. Wasn't able to grab a recording. I didn't use final shout. Iron Hide Amulet is always the culprit, but I use it very often so it might just be chance.
The artifact is re-enabled after death, but not after Death Barter. Salvaging and undoing salvage does not fix the issue. It cannot be swapped out or interacted with, only viewed as equipped.
it's been harder for me to reproduce this since howling peaks came out, although this is obviously still an issue.

Sometimes this happened to me also. Always without using "Final Shout" enchantment...
I've recorded a video viewable here.
In this run I was using Iron Hide Amulet but happened with others too.
I think is the third artifact slot bugged since is always that slot that grays out.
Happens daily to me with my third slot artifact, which is usually the iron amulet, with no final shout. When I die it resets or when I leave the level.
Can't reproduce on command, but managed to record a clip of this bug and uploaded it. Build 1.11.1.0_7392662 on PC. Hopefully this can be fixed soon.
