Video Settings Aren't Always Saved
For me the brightness option gets reset. Every now and then, when I start the game, the brightness is set to the default (moody) after setting it to +50% the former time I played.
It looks like, for at least the video settings page, there's an options state in memory that saves and updates the game, as expected, whenever you press 'done' or the 'esc' key.
However, options.txt is only saved-to when you press 'done', and not when you press the 'esc' key.

Gotta ask, do you have any mods installed?
Yes OptiFine, but I think it also happened to me when there were no mods installed. I just started Minecraft (with OptiFine installed) and the brightness wasn't reset so I assume that it only appears rarely.
Ok well if you can confirm that this happens without any mods installed, feel free to create a new issue ticket.
This happen to me very often. with no mods.
Ok, I have news regarding this. This was a problem I had last night. I turned my computer off. My internet was out for a bit because of "maintenance." When I turned my computer on, I played some minecraft, and the problem was fixed. I don't know if it was the reboot, or the internet.
I have this issue. But only in Minecraft 1.6.1.
Is this still a concern in the latest Minecraft version 14w04b? If so, please update the affected versions in order to best aid Mojang ensuring bugs are still valid in the latest releases/pre-releases.

In 1.9, it seems to only apply to the sliders (render distance, framerate, mipmap, brightness). Buttons work fine.
