I have got stuck behind fences a few times now. I kept rolling about and it eventually let me roll out. It definately needs to allow you to roll out of areas you are not supposed to be in more easily.
The torment quiver doesn't use arrows. You can pretty much go between using souls for it and back to arrows. It also goes through walls and is deadly with growing or multishoot.
From what I have seen they do. You could list your system specs or do a dxdiag and upload it but they are probably getting that information each time your game crashes.
Do you have access to another pc you could try logging in from? That would tell you if it's account or pc related. Probably going to be waiting on a fix eitherway though.
I made the same comment earlier, (see torment quiver plus other issues). There are a lot of effects that cancel each other out and there needs to at least be an unenchant item option or at the very least a time limit of say an hour to test the weapon out and realise things don't stack. I understand it could make weapons rediculously OP with all these effects and you realy only need growing and torment quiver already, but they do need to make it clearer which ones don't stack.
You need to find the lost tome before you can finish the level. If you look in the top right it tells you that. A pop up saying you can't leave without it would be helpful though.
I done a search of the internet and this seems like something you might be able to fix through unreal engine/epic games launcher.
Try downloading the epic games launcher if you don't already have it and then open it and enable debug logging in settings.
I got stuck exactly there and other places but was able to roll out eventualy. They definately either need to make it easier to get out of places you are not supposed to be or add in a respawn option. Only issue with that is it once respawned me behind a fence in a dungeon after dying.
That is windows talking. I would force the manufacturers latest driver. There is a way to do this, not sure if you need to delete the current one and then use this. I had to turn off automatic updates before because windows update driver stopped all my games working on my laptop.

If it's not that and deleting and restarting the credentials MS holds for you don't work I think I am out of things to try. Good luck though.

It's a random spawn. I'm not sure if they need to be unlocked in some order but you can bring up the map at the start of the level to see how many secret there are. You can exit and retry if you need the level with the secret map on.
I see you have a UE4 crash appearing. If it mentions missing symbols it could be that you need to update your graphics card drivers or your graphics card is struggling and you need a better one.
If you can upload a picture of the crash and the graphics card you have that would help.
Someone has reported that deleting their onboard graphics card drivers windows assigned and installing the drivers directly from the manufacturer has fixed this problem for them. It is quite common both on laptops and desktop pc motherboards to have a secondary gpu.

I did wonder what was causing this. Have to keep unequipping it to make it work again.
This thing is mental on a bow with growing enchantment btw.

As someone else has suggested I believe the issue with torment quiver may be final shout deactivated.
I recommend a bow with torment quiver on them instead of melee. Pretty much the only guarantee to kill them as they can have reflect on basic arrows.

Can you still go the long way round to the original entrance?
You need to unlock the church first and press on the fireplace. If you beat the game there is a way to gain access to the church.

If that doesn't work and you are on a laptop, look for the latest onboard drivers directly from the manufacturer.

Standard gamepass doesn't include pc games.
Are you playing in someone elses world? You need to collect it on yours if so.
Have you tried the reset xbox option via settings/console info/reset console?
Played the game solidly for days on release and disn't crash once. Have you tried a console hard reset?

Just to be clear, if you beat the boss you shouldn't need to leave the camp to find the answer you are looking for.
When you get in the church, make sure you check out the dog and fireplace.
Find and use the church fireplace in basecamp first.
That would seem to be something to complain to MS about.
Can you both play on one PC with an xbox controller or 2 and keyboard?
You can gameshare on xbox, but I doubt they will bring that to PC.

Can you try different armour and weapons to see which enchantment is causing the issue. It would be strange if cool down is causing it.

Are you playing online in single player and clicked on the fireplace to see which ones are unlocked already?
I think a lot of things appear more or less frequently depending on your power level.
I never realy worried too much about melee since the bow with growing and torment quiver is OP.
Which resets have you tried?

Reset in system settings/console info?

Are your graphics drivers all up to date?
The beta was probably less taxing om your PC. Now they have added more content your graphics card might not be up to the job.
Maybe add your system specs or s dx.diag
If you are playing on your world or even better on your own on your world and it's saying you have it then it probably is bugged.
Seen a few of these being related to graphics drivers not being up to date.
If you have onboard graphics that might need to be deleted and installed directly from the manufacturer.
You will need to either upload a picture of the crash or say exactly what it says. Lots of other solved crashes on here if you search.
People are trying to help you, but you need to throw them a bone or you are asking them to bumble about in a dark cave without a torch.
