I have marked this issue as fixed because:
- If an entity is in a portal at the end of a tick, it will be teleported in the next tick.
- The entity only has to be touching the portal for 1 tick.
I believe this issue was originally about a problem where an entity had to be touching a portal for more than one tick to be teleported. That bug is fixed.
There are still some issues that could be raised separately, with more specific repro steps:
1. If an entity is in a portal in a non-entity processing chunk, it will not teleport.
2. If an entity is moving fast, it can pass right through a portal without ever touching it and will not teleport (MC-196556)
3. Entities teleport though portals at the start of a tick (before they move) instead of at the end of a tick (after they move)

I believe that this is a duplicate of MC-196556 and/or MC-172031. If there are blocks present under the end portal, the ender pearl will hit these causing you to be teleported where it landed, therefore causing you the enter the end dimension as a result of end portal blocks being adjacently present at where the ender pearl made contact.
If there weren't any blocks present below the end portal, sometimes ender pearls would successfully enter the end dimension but without any effect (see MC-172031), and other times, simply pass through the end portal and not change dimensions as a result of it traveling at a high velocity (MC-196556).

If a nether portal is created in the End using commands, ender pearls will pass through the portal if the player hasn't seen the end credits.
To reproduce:
- Go to the end in a new world where the player hasn't seen the credits
- /setblock ~1 ~1 ~ nether_portal
- Throw an ender pearl into the nether portal
- Notice that the ender pearl goes past the nether portal rather than going to the nether as expected
- Jump into an end portal to see the credits
- Go back to the end
- Throw an ender pearl into the nether portal again
- Notice that the ender pearl goes into the portal as expected
You may need to try throwing ender pearls into the nether portal multiple times due to MC-196556.

When an ender pearl goes through the exit portal, it is teleported to the world spawn point, lands on the ground there and teleports you there instead of your bed. If you throw an ender pearl into the exit portal and you are teleported to your bed, you have experienced MC-196556, since the ender pearl hit the block below the portal, teleporting you into the portal and consequently at your bed. This bug report is therefore a duplicate of MC-196556.

This issue is about the projectile acceleration too fast, causing the portal to fail detect the projectile.
This also seems quite location dependent. Moving closer to or further away from the portal changes whether the projectile goes through the portal or not. My hypothesis is that this is because projectiles don't check if they passed through a portal between two ticks.

This is why it happens:
In Minecraft 1.16.1 and earlier versions, thrown projects checks if the OUTLINE shapes of blocks, to find potential collisions and chances to be teleported ("hit" a nether_portal block, for example)(See ThrowableProjectile.tick()). A portal has its non-empty OUTLINE shapes though it has no collision box which can stop regular entity movement, so thrown projectiles can always detect if it can be teleported no matter how high their velocity is.
But in 1.16.2-pre1, something was changed. The OUTLINE shape get no longer used in collision checks, instead, the COLLIDER shape has been used since then (See ThrowableProjectile.tick() & ProjectileUtil.getHitResult()). However, portals has empty COLLIDER shapes, so the teleporting check became dead code. And entities only stays in some seperated points, thus if the velocity of a thrown projectile entity is greater than 1 bloch per tick, it may pass through the portal if it failed to be teleported in the Entity.handleNetherPortal() method.

MC-73884, simply changing it back is not a good idea. But a addional ray trace may be introduced when the speed of the entity is higher than 1 block pre tick, just like what falling_blocks of concrete powder do.Still in the 24w33a.
